What is the Retina Vitreous Associates Patient Portal?

The Retina Vitreous Associates/CVP Patient Portal is a secure patient portal. It gives you a way to contact our office with non-urgent requests through a secure web portal. It also gives you a way to see information about previous visits and view your billing statements.

Is the Patient Portal the same as email?

No. The Patient Portal is a website that offers a secure system to message your physician's team or others at our office with questions. The Portal looks and may even feel like email. However, through this system we are able to view your questions alongside your chart and process the information you need. We can reply to you through this system with information from your health recond in a secure fashion.

What should I be able to do on the Patient Portal?

Currently, you are able to request your health record. A version of your health record will be submitted to you. If you need your entire medical record, you should contact Retina Vitreous Associates at 419-517-6599 with this request. Only key information from recent visits will be available on the Patient Portal. You will also be able to send a message to our practice. The Portal is for non-urgent requests only. Currently you are able to see upcoming appointments and your current statements via the Patient Portal. You can also pay your statement balance via the patient portal.

How long should I allow for response to my message?

Please allow 2 business days for proper routing of your message and an answer to be submitted to you.

How do I enroll and log onto the Patient Portal?

Go to, enter the pertinent information requested, including answering security questions.

How do I retrieve my user name?

You can retrieve your user name by clicking on the "Need help with your user name and password?" link on the log in page. You should select "I'm having problems signing in" and enter your personal information. You will receive an email with a link to reset your user name.

How do I retrieve my password?

Your password can be reset by entering your user name and answering the forgotten password security question on the log in page. After answering the security question, you will receive an email with a link to reset the password.

What if I cannot remember my security question answer and need to reset my password?

If you do not remember your password security question, choose the "I'm having trouble signing in" button on the log in page. This will ask you to enter your personal information. Once you complete this page, you will receive a link to reset your password.

What if I am having difficulty resetting my password?

Please contact 877-203-4935 for technical support. 

Can someone else log onto my account?

The system is secure. No one can access your account unless they have access to your user name and password. For internet safety, always choose a strong password (combination of letters, numbers, and special characters) and do not share your user name and password. If anyone tries to log into your account with the wrong user name and password, your account will lock after 4 attempts.
